Downloadable Event Purpose Form

I wanted to share a form I made a while back for any/all events we do at Northwest. We are trying to get focused and also make sure we communicate with everyone involved all the needed information. There have been times we found out that no one knew what certain events were about, why we were doing them, or what it involved. This form is here to get us focused and as a means of communication so we can all be on the same page from the office to the ministers to the elders to the members.
